Registered Nurse Case Manager – Home Health (Medway / Millis Area)

The Position:
They are seeking a full-time Registered Nurse Case Manager for the Medway and Millis area. This position is well suited for an RN with home health experience who enjoys coordinating patient care, managing treatment plans, and working collaboratively with an interdisciplinary team.
Highlights:
- Salary: $104,000 – $113,000
- Full-time position
- Weekday schedule: 8:00 AM – 4:30 PM
- 30 productivity points per week
- Potential for a four-day workweek
- One weekend requirement per quarter
- EMR: HCHB / PointCare
- Home health experience preferred
Openings & Schedule Options:
- Full-time, weekday schedule
- Standard schedule: 8:00 AM – 4:30 PM
- Potential four-day workweek
- Service area: Medway, Millis, and surrounding communities
- Weekend coverage required once per quarter
Requirements:
- Current Massachusetts Registered Nurse license
- Home health experience preferred
- Strong clinical assessment and care coordination skills
- Familiarity with HCHB / PointCare preferred
- Ability to travel throughout the assigned service area
Responsibilities:
- Conduct nursing assessments and develop individualized plans of care
- Coordinate and manage patient care throughout the home health episode
- Monitor patient progress and communicate changes in condition
- Collaborate with physicians and other healthcare professionals
- Educate patients and families on diagnoses, medications, and care plans
- Complete timely and accurate clinical documentation
- Promote patient safety and continuity of care
