About THINK Hauora

At THINK Hauora we are committed to enhancing health and wellbeing within all communities, including Māori, Pasifika, Youth, and LGBTQIA+ communities, across our rohe. We value diversity and inclusivity, and we warmly welcome individuals from all walks of life to contribute to our vision of Connecting Communities for Wellbeing.

THINK Hauora is committed to honouring Te Tīriti o Waitangi to improve Hauora outcomes for the communities we serve.

The Here Toitū Programme:

Here Toitū is a national programme delivered in partnership between the Ministry of Social Development (MSD) and Primary Health Organisations (PHOs). It supports people living with a health condition or disability to move towards employment when health issues are impacting their ability to do so.

THINK Hauora is one of four PHOs delivering Here Toitū across Aotearoa. Our local team includes a Programme Manager, Kaimanaaki (Navigators), a dedicated Work & Income Case Manager, and an Employment Broker.

The position is fixed‑term due to parental leave cover requirements and is tied to programme funding available until June 2027.

The Role of Kaimanaaki

As a Kaimanaaki, you will walk alongside whānau (participants) as they work towards improved wellbeing, confidence, and readiness for work, study, or volunteering. Using a strengths‑based approach, you will support whānau to identify their aspirations, build capability, and take practical steps toward their goals. This is a role where compassion, motivation, and problem‑solving skills truly make a difference.

You will provide caseload coverage within a supportive team environment, based at our Palmerston North office. Daytime travel within the MidCentral region is required, and a full, clean driver’s licence is essential. A company lease vehicle is available for work travel.

Supported by a Programme Manager, and the wider Here Toitū team, your role will involve:

  • Assessing whānau for programme suitability

  • Supporting whānau to improve health, independence, and self‑management

  • Co‑creating wellbeing action plans and supporting actionable steps

  • Building and maintaining strong relationships with providers, communities, and whānau

  • Assisting with work‑readiness preparation (CVs, cover letters, job search support)

Our service aims to cultivate empowerment, self‑reliance, growth, and long‑term wellbeing for whānau and future generations.

We would love to hear from you if you have:

  • A relevant tertiary qualification (e.g., Health & Wellbeing Level 3+) and/or experience in health, social services, or community settings

  • Experience working in a whānau‑centred way

  • Confidence using Microsoft Office and client management systems

  • Experience in Mental Health and/or Addiction services

  • Resilience, positivity, and a solutions‑focused mindset

  •   The ability to engage effectively across cultures, particularly with Māori and Pacific communities

  • The ability to work both independently and as part of a team

You’ll be joining a tight‑knit, values‑driven team that supports each other and celebrates the wins. This is a busy, rewarding role where you will see the positive impact of your mahi ripple out through whānau and into the wider community.

If this sounds like you, please apply with your CV and a covering letter. Applicants must be legally entitled to work in Aotearoa New Zealand.

Applications will be reviewed as they are received, with the aim of filling the position as soon as possible.
