Cover Letter 3. Resume 4. (3) Current Letters of Recommendation 5. TB Test Verification (TB skin test must be no more than 1yr old from date administered, test must include date administered and date read) OR TB Assessment that is dated within 60 days of hiring date. *To obtain a copy of your detailed credential or permit, please go to: 1. www.ctc.ca.gov 2.
