Day 5x8-Hour (08:00 - 17:00)

Assignment Details

Home skilled visits - 6-8 per day, minimum of 6 points per day, with potential for additional visits based on managerial discretion.

Requirements

• 2 years of home health experience required
• Valid state license
• BLS certification required
• Experience with assessment, cardiopulmonary/circulatory conditions, documentation, and equipment and devices such as crutches and gait belts
• Knowledge in ortho, neuro, and oncology patient care
• COVID vaccination or exemption form on file
• Flu shot or declination; mask wearing required while at work
• Must have valid driver's license and car for home visits

Responsibilities

• Conduct patient assessments and develop home exercise programs
• Educate patients and families on health management and safety
• Document patient progress and treatment outcomes
• Identify signs and symptoms of physical, sexual, and psychological abuse
• Ensure safety and injury prevention during patient visits
