Job Description

We are seeking a dedicated Inpatient Pharmacist to join our team in a 94-bed facility located in Kona, Hawaii. The ideal candidate will be responsible for providing pharmaceutical care to patients, ensuring safe and effective medication use, and collaborating with healthcare professionals to optimize patient outcomes.

Responsibilities

• Dispense medications accurately and safely to patients in accordance with their prescribed therapy.
• Review and evaluate medication orders for appropriateness and compatibility.
• Provide drug information and consultation to healthcare providers and patients.
• Monitor patient medication therapy for efficacy, safety, and adherence.
• Participate in interdisciplinary rounds and contribute to clinical decision-making.
• Ensure compliance with all regulatory and accreditation standards.
