Maternity Leave Cover, 1 year fixed term
Are you looking for a new clinical opportunity or to broadener your experience of cross sector working? We are looking for a motivated pharmacist with broad clinical experience to cover our Virtual Ward service at the Trust. This is an interesting opportunity to experience cross sector working in primary and secondary care, aiding in admissions avoidance.
This post is part of the Pharmacy Emergency Care Team, working in conjunction with our Lead Pharmacist for Frailty and Admissions.
The Virtual ward allows patients to receive care, remote monitoring and treatment they need in their own home, safely and conveniently rather than in hospital.
Main duties of the job
You will work as a member of the Virtual Ward multi-disciplinary team, supporting the safe management of medicine needs to patients being transferred to the Trust Virtual ward from an acute admission ward or the Emergency Department. You will also support patients by visiting them at home where necessary.
Working closely with clinicians, nursing staff and allied health professionals, you will provide expert clinical advice, support medicines optimisation and provide the safest care to our patients.
You will be expected to participate in the on call, weekend, Bank Holiday and late duty rotas.
