- Build and manage an adult patient panel in a 100% outpatient Primary Care setting.
- Assess patients, develop individualized care plans, and coordinate evaluation or specialty referrals with the clinical team.

Requirements
- Family Nurse Practitioner training
- Unrestricted Connecticut license or eligibility
- At least one year of Primary Care or relevant outpatient experience
- Current CV required
