We are looking for a confident, experienced Health Improvement Practitioner (HIP) who can step straight into a busy general practice environment and make an immediate impact.

This role is best suited to someone who is already working as a HIP (or has done so recently) and understands the pace, expectations, and clinical flow within primary care. If you’re looking for a role where you can focus on delivery, not onboarding, this is it.

Why This Role Stands Out

Immediate start – step straight into an established service

Full-time/part time opportunities – consistent hours and stability

Work within integrated primary care teams

Make a real difference to whānau wellbeing every day

Join a Pacific-led PHO driving equity and innovation

Supportive, well-connected clinical and peer network

About the Role

As a HIP, you will deliver brief interventions and behavioural health support within general practice, working side-by-side with GPs, nurses, and health coaches.

You will be:

Providing same-day, accessible support to patients/whānau

Using brief intervention models to improve mental health and wellbeing

Supporting lifestyle and behavioural change

Contributing to a fast-paced, team-based care model

This is a role where your ability to engage quickly, think on your feet, and deliver effectively will be highly valued.

While our offices are based in Manukau, we are currently seeking Health Improvement Practitioners to work from GP practices located in Avondale, Māngere, Flat Bush, Papakura, and Manurewa.
