The CSW/LSW Social Worker provides support, counseling, and case management services to individuals, families, and groups. We are seeking a dedicated and compassionate CSW/LSW Social Worker to join our team of professionals. The ideal candidate will be passionate about working with the senior population and committed to providing high-quality support and care.
Currently Hiring for Part Time or Per Diem
Compensation:
$35 which is negotiable and can be discussed further during the interview.
Key Responsibilities
Conducting admission and ongoing assessments, including documentation
Assisting residents with adjustment to the facility
Completing and maintaining MDS 3.0 documentation
Developing and participating in care planning
Assisting with Medicaid applications and other financial matters
Advocating for residents’ needs and rights
Providing crisis intervention when necessary
Coordinating and supporting discharge planning
Qualifications:
Must hold a valid New Jersey Social Work Certification (CSW/LSW) in good standing
Strong organizational skills with the ability to multitask effectively
Passion for working with the senior population
Excellent writing and documentation skills
Experience working in a Nursing home Facility is preferred
