  • To be their clients on-going first person to contact if they have a question around their health care.

We are after an individual with the following skills and attributes:

  • Basic understanding and knowledge of health conditions (hypertension and diabetes in particular).

  • Having lived experience in the health sector is a bonus.

  • Good caseload management - you will be responsible for managing your caseload, while working collaboratively with the wider team.

  • Excellent communication skills.

  • Prior training or certificates relevant to the role are required.

  • Will have good IT skills as you will be required to enter clinical notes into the Patient Management System in a timely manner.

  • Good understanding of behavioural change techniques.

  • Experience in skills of problem solving, self-management, goal setting and creating action plans.
