Registered Nurse Case Manager – Home Health (Natick Area)

The Position:
They are seeking a full-time Registered Nurse Case Manager to support patients in the Natick area. The ideal candidate will have home health experience and be comfortable managing patient care, coordinating services, and maintaining accurate clinical documentation.
Highlights:
- Salary: $104,000 – $113,000
- Full-time position
- Weekday schedule: 8:00 AM – 4:30 PM
- 30 productivity points per week
- Potential for a four-day workweek
- One weekend requirement per quarter
- EMR: HCHB / PointCare
- Home health experience preferred

- Standard schedule: 8:00 AM – 4:30 PM
- Potential four-day workweek
- Service area: Natick and surrounding communities
- Weekend coverage required once per quarter
Requirements:
- Current Massachusetts Registered Nurse license
- Home health experience preferred
- Strong clinical assessment and care coordination skills
- Familiarity with HCHB / PointCare preferred
- Ability to travel throughout the assigned service area
Responsibilities:
- Assess patients and develop individualized plans of care
- Manage and coordinate patient care throughout the home health episode
- Monitor patient conditions and communicate changes to physicians and the care team
- Perform skilled nursing assessments and interventions as needed
- Coordinate services with patients, families, physicians, and other healthcare professionals
- Maintain accurate clinical documentation in the EMR
- Support patient safety, continuity of care, and achievement of clinical goals
