RN – Case Manager – Hospice

Schedule

Days: 8:30am-5pm (Schedule can be flexible based on patient's availability). One weekend per month required.

Assignment Details

Holiday coverage is required.

Requirements

1 Year of Hospice Nursing Experience
General Certifications (BLS/BCLS)

Responsibilities

Admit patients to service and assess their physical, psychosocial, environmental, safety, and developmental needs.
Develop individualized plans of care based on assessments.
Document observations and interventions related to patients' environments.
Coordinate care with a multidisciplinary team.
Participate in the quality assessment performance improvement program.
Conduct follow-up visits as needed.
