The Position:
They are seeking a full-time Registered Nurse Case Manager to serve patients in the Worcester area. The ideal candidate will have home health experience and strong skills in patient assessment, care planning, and coordination.

- Salary: $104,000 – $113,000

- Weekday schedule: 8:00 AM – 4:30 PM
- 30 productivity points per week
- Potential for a four-day workweek
- One weekend requirement per quarter
- EMR: HCHB / PointCare
- Home health experience preferred

- Service area: Worcester and surrounding communities
- Weekend coverage required once per quarter
Requirements:
- Current Massachusetts Registered Nurse license
- Home health experience preferred
- Strong clinical assessment and care coordination skills
- Familiarity with HCHB / PointCare preferred
- Ability to travel throughout the assigned service area
Responsibilities:
- Perform nursing assessments and develop individualized plans of care
- Manage and coordinate patient care throughout the home health episode
- Monitor patient conditions and report changes to the appropriate healthcare professionals
- Coordinate care with physicians, patients, families, and the interdisciplinary team
- Provide patient and family education
- Maintain accurate and timely clinical documentation
- Promote safe, effective, and coordinated patient care
