Making a meaningful difference in the health and wellbeing of Pacific communities.
The Fono is growing its Health Coach team and has multiple opportunities available across both our LagiOla and Outreach teams.
We are looking for passionate, people-focused individuals who want to make a genuine difference in the lives of Pacific individuals, whānau and communities.
Whether your strengths lie in health and wellbeing coaching, community engagement, outreach, health promotion, supporting whānau or connecting people with the right services, we want to hear from you.
About the Role
Health Coaches work alongside individuals, whānau and communities to strengthen health and wellbeing, build self-management capability and support people to achieve goals that are meaningful to them.
Depending on the team you join, your work may involve a combination of one-to-one support, community engagement, outreach, health promotion, group activities, health coaching and service navigation.
Across both LagiOla and Outreach, you will:
Build trusting and respectful relationships with clients, whānau and communities
Support people to identify their strengths, priorities and meaningful health and wellbeing goals
Provide practical support to help people make sustainable lifestyle and behaviour changes
Build health knowledge, confidence, self-management skills and independence
Connect people with appropriate health, mental health, social, cultural and community services
Participate in community outreach, events and health promotion activities
Support or facilitate group sessions and education where appropriate
Work collaboratively with The Fono's multidisciplinary teams and community partners
Maintain accurate and timely client records and follow-up
Contribute to quality improvement, service evaluation and positive client outcomes

You will work in a way that protects dignity, mana and choice while helping to reduce barriers to accessing health and social services.
What You Will Bring
Warm, approachable and genuinely passionate about helping others
Strong communicators and active listeners
Able to build trusted relationships with people from diverse backgrounds
Comfortable supporting people through goal setting, coaching and behaviour change
