Therapy/Rehabilitation – PT Home Health

Schedule

Day shift, 5 days a week, 8-hour shifts (08:00 - 17:00). Every third weekend required.

Assignment Details

Skilled home health visits, 6-8 visits per day (minimum of 6 points per day with potential for additional points at manager's discretion).

Requirements

• 2 years of experience in a home health setting required.
• Valid state license and BLS certification required.
• Must have a valid driver's license and own vehicle for home visits.
• Must be COVID vaccinated or have an approved exemption.
• Flu shot must be received or declination submitted; mandatory mask wearing at all times while at work.
• OASIS training preferred.
• Experience with various equipment and devices such as crutches, gait belts, and oxygen delivery systems.

Responsibilities

• Conduct patient assessments and develop home exercise programs.
• Document patient care and progress in compliance with regulations.
• Educate patients and families on medications, safety, and signs of abuse.
• Ensure the safe use of medical equipment during therapy sessions.
• Monitor and document patient response to interventions.
• Work collaboratively with interdisciplinary teams to optimize patient care.
